what's more

housing support

We work in partnership with the private landlord to ensure that the individual person receives the right support to manager their tenancy agreement. This gives us a unique position to proactively liaise with their landlord about regarding their property and maintenance and or any repair issues they may have.

getting support

A Care Manager will usually contact us on the person’s behalf with a copy of the individual’s care plan to review. This provides us with vital information regarding the needs of the person.

Before we offer a service to any individual we will arrange to carry out an assessment of need. This is normally carried out in the person’s own home and doing this visit we will be able to ascertain what sort of support the person wants and needs and whether we can provide the right service.

At the visit we will ask questions about the individual’s health, support needs with personal care, mobility, daily living skills for example: cooking, shopping, medication and the person’s interests and hobbies. We will also ask what support is needed to keep safe and manage their life for example: help with post, safely at home.

We encourage people to invite their family, friend with them whilst we complete the assessment.

We will bring a brochure and Handbook for the person to look at – this helps them know about Infinity Services (UK) Ltd and what services we offer.

We will invite the person to come and visit their potential new home. We will answer any questions the person may have about living in their own home during this time.

If the person is happy and we can provide the service, we will work with the Care Manager to agree how much support is needed and the related cost.

Once the person has begun using our services we will work them to complete a Person Centred Plan, which includes what the person likes and need. We encourage individuals to work with their family and friends to help support them to do this.

We will then develop the support plan, which informs the staff how to give the support needed.

Our staff will review the individual support plan each month to ensure that the person’s needs are being met effectively with positive outcomes.

The support plan belongs to the individual and will always be kept in their own home. Our staff will also keep a copy which will be located in the office.

Everyone who uses our services receives copies of our most important policies and procedures. These documents inform our staff what is expected of them.

All service users have the freedom of choice in choosing their preferred care provider if they do not wish Infinity Services (UK) Ltd to support them on a day to day basis.
